Medical Social Worker

Performs Psychosocial assessments, evaluates the patient/family coping, social service needs, caregiving plans, along with the family’s financial status and insurance coverage.
Conducts evaluations as assigned and communicates with the clinical team and in case management meetings.
Crisis Intervention
Provides counseling to include clinical assessment, consultation of treatment plan, plan of care, long-term care planning, and community resource outreach.
